Personalised Stratified Follow Up for prostate cancer

TrueNTH Supported self-Management is a cost-effective programme for personalised stratified follow up that delivers improved outcomes for prostate cancer patients and frees up capacity in the follow up pathway. From 2015 researchers from the University of Southampton, funded by the Movember Foundation and delivered in partnership with Prostate Cancer UK, trialled the TrueNTH Supported Self-Management programme in five NHS Trusts. The programme moves follow up care from outpatient clinics to monitoring patients remotely, meaning eligible men do not need to attend routine face to face appointments unless something of concern comes up.

Online patient service

Access to an online portal that allows patients to check test results, complete assessments, view patient information and message their clinical team.

Remote motoring of their condition by the clinical team

A PSA tracking system that can alert the clinical team to missed tests and results that fall outside the personalised safe ranges, ensuring no man is lost to follow up.

A band 4 support worker

The support worker acts as a key worker during their follow up care. This person is solely dedicated to the follow-up care pathway and critical to ensuring it’s delivered safely and effectively.

Workshops

A 4-hour supported self-management workshop with a group of 8 to 10 men to develop their knowledge, skills and confidence to manage their prostate cancer follow up. This session also introduces the patient to the remote monitoring IT portal. Online learning to support you to deliver these workshops will soon be available.

During the trial of supported self-management in NHS trusts the evidence showed;

  • patients reported equally good outcomes when monitored remotely compared to havingappointments for their follow-up care
  • satisfaction levels with the model of care were high.
  • the programme met NICE cost-effectiveness criteria
  • capacity issues were addressed by replacing a one-size-fits-all model, with a more personalised approach for men who are progressing well
  • the patient had a positive role in their recovery and looking after their health and wellbeing
